What Is My Living Voice?
My Living Voice is a secure online platform that makes documenting an advanced healthcare directive quick and simple. The program will guide you through the process to document your preferences for care during a medical crisis, name an individual to speak for you if you become unable, and describe what live-extending care you want. It is a free resource included with the health benefits for our Medicare and Duel Eligible Special Needs Plan members. Currently applicable to Medicare plans only.
Account Creation and Access
To create and access a My Living Voice account, go to centene.mylivingvoice.com
- At the bottom of the page, Click Here
- Click one of the blue Start my Advance Directive buttons on the page.
- If you have not created an account yet, sign up.
- Fill in the blanks and agree to the terms and conditions
- The My Living Voice advance directive has three parts:
- Healthcare Proxy
- Personal Values Statement
- My Goals of Care and My Treatment Wishes
Sharing My Living Voice Advance Directive
Once you completed your advance directive, you have the option to share it with your healthcare proxy, doctor, health plan, or anyone else that you deem necessary.
- When you log back into your account, you will come to your advance directive dashboard.
- Click:
- Then, choose from the drop-down list who you would like to share your advance directive with.
- Send.
- Click Download My Advance Directive to complete your advanced directive and learn the requirements specific to your state to make your document legal.
